Why Diagnosis Matters
A formal autism diagnosis in India unlocks school accommodations, disability certificate (for government benefits), right to extra exam time in CBSE/ICSE/state boards, access to government therapy centres, and clarity for the family. It also gives parents the language to advocate for their child.
Many families struggle with the question: "Does my child actually have autism, or is this just a phase?" A professional assessment answers this question definitively and gives you a roadmap.
Step 1: Start with Your Pediatrician
Your first visit should be to your child's pediatrician. Describe what you are observing in specific, concrete terms: "He does not respond to his name 8 out of 10 times," "She lost the 10 words she had at 14 months," "He lines up toys for 30 minutes instead of playing with them."
Ask for a referral to a developmental pediatrician or child psychiatrist. General pediatricians typically do not do autism assessments themselves — they refer onward.
Also ask the pediatrician to:
- Rule out hearing loss (get a hearing test / BERA)
- Check for vision issues
- Administer the M-CHAT screening questionnaire if the child is 16–30 months
Step 2: The Developmental Pediatrician or Child Psychiatrist
This doctor conducts the actual diagnostic assessment. What they do:
History taking (45–90 minutes):
- Detailed pregnancy and birth history
- Developmental milestones (when did the child sit, walk, first say words)
- Current behaviors, strengths, and challenges
- Family history of autism, ADHD, anxiety, language delays
Direct observation of the child:
The doctor plays with and observes the child to assess social communication, play, repetitive behaviors, and sensory responses. For formal assessment, tools used include:
- ADOS-2 (Autism Diagnostic Observation Schedule) — gold standard internationally, available at NIMHANS and some private centres
- CARS (Childhood Autism Rating Scale) — widely used in India, takes 30–45 minutes
- 3di or ADI-R — structured parent interview
Psychological testing (sometimes):
A psychologist may separately assess cognitive ability (IQ), adaptive behavior (Vineland scales), and language level. This is not required for diagnosis but helps with therapy planning.
Step 3: The Diagnostic Report
After assessment, you receive a written report including:
- DSM-5 diagnosis (Autism Spectrum Disorder, Level 1/2/3)
- Any co-occurring conditions (ADHD, intellectual disability, anxiety)
- Recommendations for therapy
- Severity level and functional description
Keep multiple copies of this report. You will need it for school, disability certificate, and any future evaluations.
Where to Get Diagnosed in India
Government / low cost:
- NIMHANS Bangalore — waiting list can be 6–12 months; nominal fees
- AIIMS Delhi — child psychiatry department; long wait
- Government medical colleges — variable quality; check if they use CARS or ADOS
Private centres (faster, higher cost):
- Developmental pediatricians in major hospitals (Apollo, Fortis, Rainbow, Manipal)
- Developmental pediatricians in private practice — search "developmental pediatrician [your city]"
- Specialized autism assessment centres in Mumbai, Delhi, Bangalore, Hyderabad, Chennai
Cost:
- Government: ₹500–3,000 for full assessment
- Private: ₹5,000–25,000 depending on tools used and doctor
Time from first appointment to report:
- Government: 2–6 months (waitlists are long)
- Private: 2–6 weeks
Step 4: Getting the Disability Certificate
After diagnosis, apply for a disability certificate at your district hospital or government medical college. This certificate is issued by a government doctor (not your private doctor's report alone). Bring:
- Private diagnosis report
- Recent photo IDs
- Child's birth certificate
The certificate gives your child rights under the RPwD Act 2016 including educational accommodations and government scheme benefits.
After Diagnosis
Start therapy as early as possible. Priority order:
1. Speech and Language Therapy
2. Occupational Therapy
3. Applied Behavior Analysis (if recommended)
4. Special education support
Do not wait for the "perfect" therapy setup. Start with whatever is accessible now.
