What Is Play Therapy?
Play therapy uses play — a child's natural language — as the medium of therapeutic intervention. For autistic children, two main approaches are used in India:
Floortime/DIR (Developmental, Individual Difference, Relationship-based): Developed by Dr Stanley Greenspan. The therapist and parent get on the child's level, follow their lead, and gradually expand the interaction circle. Highly relationship-focused. Strong evidence for improving social-emotional engagement.
Filial Therapy / CPRT (Child-Parent Relationship Therapy): Training parents to conduct play sessions at home. Evidence shows parental play skill training has similar outcomes to direct therapy, with the added benefit of generalizing across settings.
Social Skills Play Groups: Structured play with 2–4 peers, facilitated by a therapist. Teaches turn-taking, sharing, imaginative play, and conversation through practice in a safe setting.
How It Differs from ABA and Speech Therapy
| | Play Therapy | ABA | Speech Therapy |
|---|---|---|---|
| Focus | Relationship, play, emotion | Behavior, skill acquisition | Communication, language |
| Structure | Child-led | Therapist-directed | Mix |
| Environment | Play room / home | Clinic / home | Clinic |
| Evidence base | Good for social-emotional | Strong for behavior | Strong for language |
For most autistic children, a combination is ideal: ABA or speech therapy for skill-building, plus play therapy or Floortime for social-emotional connection.
Which Children Benefit Most
Play therapy is particularly helpful for:
- Children who resist or shut down in structured therapy
- Children with strong sensory and play interests that can be leveraged
- Children where the parent-child relationship is strained due to caregiving stress
- Children with predominantly social-emotional rather than behavioral challenges
Finding Play Therapists in India
Play therapy as a formal specialization is still emerging in India. Look for:
- Floortime-trained therapists (DIR/Floortime certification from ICDL or equivalent)
- Developmental therapists at autism-specific centers
- Child psychologists with play therapy post-graduate training
Centers known to offer Floortime or developmental play approaches: NIMHANS Bangalore, Action For Autism, Vidya Sagar Chennai, Tamanna School Hyderabad.
Cost: ₹800–2,500 per session depending on city and therapist.
Practice Play Therapy at Home
Floortime can be done by parents without formal training. The core principle: 20–30 minutes daily, join your child's world on the floor, follow their lead, add one small elaboration to expand the interaction.
