How to Get a Disability Certificate for Autism in India (UDID Card Process)

Step-by-step guide to getting the UDID disability certificate for autism in India — documentation, process, which doctor certifies, and what benefits it unlocks.

What is the UDID Card?

The Unique Disability ID (UDID) card is a national disability identity card issued by the Government of India under the Swavlamban scheme. It creates a single, unified database of all people with disabilities in India and replaces the older state-level disability certificates with a standardised national document.

The UDID card is essential for accessing virtually all government schemes, concessions, and rights available to people with disabilities in India.

Why the UDID Card is Essential for Autistic Children

With a UDID card, your child is entitled to:

  • Education: Reservations in government educational institutions, access to inclusive education support under RPWD Act
  • Travel: 50–75% concession on Indian Railways (varies by class); some state governments offer free bus travel
  • Tax benefits: Parents of disabled children can claim deductions under Section 80DD and 80DDB of the Income Tax Act
  • Government employment: Reservation in government jobs (relevant when your child grows up)
  • Welfare schemes: Priority access to government disability welfare programmes
  • Insurance: Some health insurance policies have provisions for disability
  • Priority in government services

Without a certificate, families often cannot access these rights even when their child clearly qualifies.

Step-by-Step Process to Get the Autism Disability Certificate

Step 1: Get a Medical Assessment

Visit a government hospital with a psychiatry or neurodevelopmental department. For autism, the certifying authority must be a medical professional trained in assessing autism:

  • Psychiatrist (most commonly certifies)
  • Neurologist (for children with comorbid neurological conditions)
  • Paediatrician in some states (check local rules)

AIIMS, government medical colleges, and district hospitals (with psychiatry departments) are valid certification centres. Private doctors can also certify in some states — check with your state's Swavlamban nodal officer.

Bring your child's existing diagnosis reports, psychological assessments, and any other documentation you have.

Step 2: Get the Disability Assessment Form Completed

The doctor completes Form 1 (medical certificate form) specifying the nature of disability (autism spectrum disorder), percentage of disability, and whether the condition is permanent.

For autism, the disability is typically certified as permanent. The percentage awarded reflects severity, but for UDID purposes, even a lower percentage (e.g., 40%) qualifies for most schemes. A 40% or higher disability certificate is required for most benefits.

Step 3: Apply Online at swavlambancard.gov.in

1. Visit swavlambancard.gov.in

2. Click "Apply for UDID Card"

3. Fill in personal details, upload the completed medical certificate, passport photo, and identity proof (Aadhaar)

4. Submit the application and note the application number

Step 4: Verification and Card Issue

The application goes to your district disability welfare office for verification. The UDID card is typically issued within 30–60 days. You can track status on the Swavlamban portal with your application number.

Documents Required

  • Completed medical certificate (Form 1) signed by authorised doctor
  • Aadhaar card (child's or parent's if child is below 5)
  • Passport-sized photograph
  • Birth certificate (for children)
  • Existing diagnosis reports (helpful, not always required)

How Long Does It Take?

From first appointment to receiving the card: typically 2–4 months. Delays are common in high-volume government hospitals. Following up with your district disability welfare office by phone or in person can speed things up.

What Benefits the Certificate Actually Unlocks

The disability certificate alone does not automatically deliver benefits. You must actively apply to specific schemes using the certificate. Key schemes to apply for:

  • National Trust schemes (nationaltrust.gov.in) — residential, respite, and caregiver training support
  • NIMHANS and state mental health authority services — subsidised or free therapy
  • State-specific welfare allowances — many states give a monthly disability allowance
